5. Record (Evidence of Accomplishment)
Record what I have learned and how it has helped me on the job.
Look for tangible evidence such as certificates of completion, copies of articles you published and/or presented. Complete the seventh column in the Continuous Learning Portfolio.
Also, you may wish to copy all your certificates, conference attendance receipts, workshop registration forms and any other evidence of completion of continuous learning activities and place them in a safe place for future reference as these documents will help you when completing your Continuous Learning Portfolio.
Note you may find, as each year progresses, that you have an opportunity to develop skills in areas you did not anticipate when you put your learning goals and action plan together. For example, restructuring in your facility may require you to learn about skills historically carried out by another health care profession, such as haemodynamics. Or a workshop offered in your facility on team-building techniques may be an opportunity to enhance your skills in this area. Your portfolio should be a living document. It provides you with the flexibility to add new information and record it as it becomes available.

- Record of Professional Readings
Readings related to your profession
Provide specific information on any books, articles or research papers you have read. The length of time it took to read them has to be reported in terms of hours. This standard format ensures consistency in evaluating activities. For example, if you engaged in two hours and 30 minutes of Professional Readings, over a period of three days, record this as 2.5 hours. The Quality Assurance Committee is interested in the length of time that you were engaged in a continuous learning activity and how it relates to your learning goals.
Also, remember to write down how it has helped you on the job.

- Record of Professional Development
Attendance at seminars, conferences, training courses
Provide specific details of attendance at seminars, conferences and training courses, such as CPR.
You may use one form to record attendance at a conference that was more than one day in length. Each full day attended counts as seven hours, therefore, a 3-day conference equals 21 hours of continuous learning activities. Be sure to include the full name, location and dates of the seminar, conference or training course that you attended. Write a brief description of the content in the “Description of Content” area on the form. Remember to include a brief description of how it helped you on the job.

- Record of Learning from Experts
Attendance at rounds, tutorials or cd, video and/or internet learning
Provide specific information of learning from experts. This can include attendance at rounds, tutorials, CDs or videos, and/or internet learning. The internet provides a source of professional material. You can refer to an internet address or web site under “location of activity.”

- Record of Professional Achievements
1. Seminar and/or conference presentation, teaching experience
If you prepared a seminar, workshop or presentation for a conference or at your place of practice, record the number of preparation hours and the number of hours for the presentation itself. Provide some detail. Note type of occasion, name of association/institution/organization, subject and objectives of the presentation, audience, length of preparation time, length of presentation, and date of the presentation.
2. Publication of a journal article
If you wrote an article for a journal, record the number of hours for preparation. Provide some detail. Note type of publication, name, volume and number of the journal or magazine, the title, subject of the publication, date of publication, publisher and length of time to complete.
Remember to write down how the presentation or writing the article helped you on the job.

- Summary of Self-Assessment Profile and Continuous Learning Portfolio
This is the last form you fill in, as it provides a summary of the total hours for each type of continuous learning activity you engaged in that year.
If you are randomly selected to submit your QA Self-Assessment Profile and Continuous Learning Portfolio to the Quality Assurance Committee, remember to send in this form, and place it at the front. Learning activities should be reported in hours, not minutes. This standard format ensures consistency in evaluating activities. For example, if you engaged in two hours and 30 minutes of Professional Readings, over a period of three days, record this as 2.5 hours. The Quality Assurance Committee is interested in the length of time of the learning activity and how it relates to your learning goals.
Any learning activity that does not fit into the five specified categories can be totalled under “Other”, with an explanation regarding the activity and reference to the appropriate form you used to record this activity.
Remember to write your final learning goals on this form. Your learning goals may have changed during the course of the year. This form is filled in at the end of the calendar year. You should record your final learning goals to which your learning activities link.
